Most US-licensed 503A compounding pharmacies label compounded semaglutide with a 28-day beyond-use date after the vial is first punctured. Until that first puncture, the labeled beyond-use date typically ranges from 60 to 90 days depending on the formulation, concentration, and the pharmacy's validated stability data.
These windows reflect industry-standard practice for compounded sterile injectables — not a universal pharmaceutical truth about the semaglutide molecule itself. Brand-name FDA-approved Wegovy® and Ozempic® are stable for longer windows under their manufacturer's labeled conditions (Wegovy®: stable in fridge from time of manufacture to expiration; in-use stability after first puncture: 28 days at 36-46°F or up to 56 days at room temperature per FDA label). The 28-day post-opening limit on compounded versions is a conservative pharmacy practice rather than a known molecular instability.
Compounded semaglutide is not FDA-approved and is not therapeutically equivalent to FDA-approved branded products. The FDA does not review compounded medications for stability before they reach patients. Always defer to your specific medication label and your pharmacy's instructions.
What "stable" actually means here
When a pharmacy assigns a 28-day beyond-use date, they are saying their validated testing supports the medication retaining its potency, sterility, and structural integrity for that period under proper storage. After the labeled window, the medication is not necessarily "expired" in the sense of dangerous — but the pharmacy can no longer guarantee its potency or sterility.
Two things degrade over time:
Potency: The semaglutide molecule can slowly degrade with heat, light, or repeated freeze-thaw cycles. Degradation reduces clinical effectiveness but does not typically produce harmful byproducts.
Sterility: Once a multi-dose vial is punctured for the first time, atmospheric contamination risk increases. Pharmacies factor this into the beyond-use date — the longer a vial stays open, the higher the microbial contamination risk.
Storage rules that matter most
Compounded semaglutide should generally be:
- Stored refrigerated at 36-46°F (2-8°C) — the same range as the original Wegovy® / Ozempic® storage condition
- Kept in the original sealed packaging until first use, then stored upright in the original vial after first puncture
- Kept away from direct light (refrigerator door is fine; window-facing shelf is not)
- Never frozen — freezing destroys the molecular structure and the medication cannot be safely used after thawing
- Not stored near the freezer compartment of a fridge, where temperatures can drop below freezing intermittently
- Used within the labeled beyond-use date printed on your specific medication
What to do if it has been at room temperature
Brand-name Wegovy® / Ozempic® is FDA-labeled as stable for up to 56 days at room temperature (up to 86°F / 30°C) after the first puncture. Compounded semaglutide does not have validated room-temperature stability data in most cases. The conservative rule that most 503A pharmacies and providers apply:
- Less than 8 hours at typical room temperature (60-75°F): the medication is likely fine to continue using — but consult your pharmacy if you are unsure
- Between 8 and 24 hours: contact your pharmacy before using — most will advise discarding and replacing
- More than 24 hours at room temperature, OR any time above 90°F / 32°C: discard and contact your pharmacy for a replacement. Do not use.
- Has been frozen at any point: discard. Do not thaw and use. Contact your pharmacy.
- Looks cloudy, has visible particles, has changed color, or has unusual odor at any point: discard. Do not use.
Travel and shipping considerations
Most US-licensed 503A pharmacies ship compounded semaglutide with a validated cold-chain protocol — a chilled gel pack inside an insulated mailer, calibrated to maintain refrigerated temperature for the expected delivery window. Patients should refrigerate the medication promptly upon receipt.
For patient travel:
Short trips (1-3 days): A small insulated travel pouch with a gel pack is generally sufficient. Many GLP-1 patients use commercial travel cases (BÄCC, Diabetic Travel Bag, FRIO insulin wallets) designed for refrigerated injectables.
Longer trips (3+ days) or international travel: Plan for mid-trip cold storage — either at a hotel mini-fridge (request guaranteed refrigeration when booking), via a temperature-controlled mailer if mailing to your destination, or by timing your dose schedule so you do not need to carry the medication out of refrigeration for more than a few hours at a time.
Flights: Carry medication in your carry-on bag with a TSA-compliant gel pack (frozen ice packs are not always permitted; pre-cooled gel packs above frozen are typically allowed). TSA allows medically necessary medications in larger-than-3.4oz volumes when declared at the security checkpoint. Bring your prescription label or pharmacy paperwork in case of inspection.
How storage rules differ across compounding pharmacies
Different 503A compounding pharmacies use different preservative systems, concentrations, and beyond-use validation protocols. As a result, two compounded semaglutide products can have different labeled storage windows even though the active molecule is the same.
